First-Year Application Checklist
- Online Common Application (free) or paper Common Application ($50 fee), with essay attached.
- Common Application Supplement
- School Report form
- Teacher Recommendation form, completed by a teacher who has taught you in an academic course (English, mathematics, science, foreign language, history, or social studies). Portfolio Path applicants must submit three academic teacher recommendations.

Official high school transcript.
Early Action (November 1) candidates must submit a transcript including grades from 9th through 11th grade and a complete list of courses in which they are enrolled during their senior year.
Regular Decision (February 1) candidates must submit the same as above plus grades from the first marking period of the senior year. (A copy of your report card is acceptable.) Regular Decision applications may be delayed until grades from the first marking period are received. - Seventh-semester transcript. All first-year applicants must submit this for review by the Admissions Committee as soon as it is available.
- SAT and/or ACT (timed writing portion optional) scores. These are considered official if reported on the official high school transcript or received directly from a testing agency. Lewis & Clark's SAT code is 4384 and ACT code is 3464. Students applying via the Portfolio Path may choose to have their counselors remove the scores from their records before submission to Lewis & Clark, and should indicate this in the top section of the Counselor Report form.
