In order to request that the ECLS complete and send an e-C-2 form to TSPC on your behalf, please complete and submit the form below. Some or all of the information included on this e-ROLR form will be released to the Oregon Teacher Standards and Practices Commission (TSPC) on the electronic C-2 form. You will receive a hard-copy of the completed electronic C-2 form in the mail--you may retain this hard-copy for your records or submit it to TSPC with the rest of your application packet.
