2010 Summary of Material Modifications for Pioneer Medical/Dental Participants

To all Pioneer Medical/Dental Plan Participants:

Lewis & Clark College is required to distribute a summary of benefit plan changes annually to you and/or your beneficiaries who are eligible for coverage under the Pioneer Educators Health Trust. Human Resources is furnishing the Summary of Material Modifications (SMM) to you electronically although please note you have a right to request and obtain a paper version of the document at no charge. Please keep a copy of the SMM with your other benefits information.

The changes were originally described in this spring’s open enrollment materials.
