If you have never met with someone in our office, do not use this form.  If you’re not sure what documents we have on file, please contact us.

If you have already set up an official Notice of Disability or Letter of Disability, you may request notetakers for this semester by completing the form below.

If you would like to request to send your Notice or Letter of Disability to your professors or schedule an exam in our office, use the links in the right-hand menu.
