IMS Equipment Request Form


Hi there. All staff, faculty, and students of Lewis & Clark College have access to our swell collection of AV equipment. Use this handy form to let us know what you desire. We will confirm your request by email. Please note this is a request form only; it does not guarantee that the equipment will be available.

If you need to make reservations for AV setup for an event, don't use this form; instead, call Linda at 503-768-7293 or Patrick at 503-768-7294.

Time and date of pickup is next. You can borrow equipment overnight or over a weekend. Fall and Spring we're here 9:00 AM-5:00 PM; Summer hours are generally M-F 9:00 AM-2:00 PM.

Important Note: If you are more than 1 hour late for pickup, your reservation may be cancelled so the equipment can be made available for others. Late returns mean an immediate late fee and may result in loss of borrowing privileges. See store for details!
