COVID-19 Community Reporting Form

The purpose of this form is to create an avenue where LC community members can report violations of LC’s COVID-19 expectations or the COVID-19 Mitigation Policy. Our hope is that community members will address minor health guideline violations with each other through informal conversations, but this form is available should they be unable to do so or if the violation is more egregious.
