What Students Need to Know About Uploading Proof of COVID Vaccination

After your last COVID vaccination, students should take ALL of the following steps to record proof of vaccination with the College

Dear Students:

Congratulations on making it to the end of the semester!

Many of you have already sought out COVID vaccine. Good for you! Be sure to keep your vaccination record in a safe place that you won’t forget. After your last COVID vaccination, students should take ALL of the following steps to record proof of vaccination with the College.

  1. Login to the Health Information Portal.

  2. Click on “Immunizations” at the top of the screen, then “Enter Dates” and follow the directions to enter the dates of your vaccine.

  3. Upload an image of your COVID vaccination card.

  • From the Menu Bar at the top of the Home Page in the Portal, select “Upload.”

  • Select “COVID-19 Vaccination Record Card” as the document you are uploading.

  • Click “Select File” to browse for the file on your computer. Highlight the file and select “Open.”

  • Click the blue “Upload” box.

PLEASE NOTE:

  • Make sure your name and date of birth are on the document.

  • Only Images (.gif, .png, .tiff, .tif, .jpg, .jpeg) and Documents (.txt, .pdf) are allowed.

  • File size must be smaller than 4MB.

Directions of how to request medical and non-medical (e.g., religious) exemptions will be posted to the COVID Vaccination Policy FAQ page later this month. We encourage you to check out that webpage for any questions that you have, and to e-mail covid19info@lclark.edu if you have questions that are not answered there.
