To set up an account for access to Colleague, you will need the access request form filled out and signed by your supervisor. The data custodian for the modules you will be accessing will need to provide approval for your access as well. Follow the instructions on the following form and also do the on-line training for FERPA.
Please print this two-sided as the signatures must both be on the same piece of paper.
