Waiving L&C Insurance

Waiving the insurance is a tricky matter. It has to be done online and the website is really designed for US residents as opposed to international students. (We have complained about this many times, but no one at the insurance company seems to inclined the change it).

To waive the insurance: (Make sure you have the student’s insurance card with the insurance address and telephone number).

  1. Go to:
    http://www.aetnastudenthealth.com/stu_conn/student_connection.aspx?groupid=890438
  2. Click on “Continue” at the bottom of the page.
  3. Click on the box that states “I understand and agree”¦.”
  4. Type in the student’s L&C ID number (on their ID card or ask ISS staff).
  5. Put in the date of birth. (Ask the student their birth date).
  6. Click on “Continue” at the bottom of the page.
  7. A new page should appear with the student’s home address and L&C email.
  8. If it appears to be the correct student, click “continue” at the bottom of the page.
  9. Click all of the “Yes” boxes (after confirming with the student that the info is true). (If you click a “No” box, you cannot get a waiver).
  10. Then fill in the info in the blanks:
    • Insurance company: Only US insurance companies are listed. So, choose “Other” at the very end of the list.
    • For “Policy Holder Name,” write in the student’s name.
    • For address of insurance company, write in the address, but make sure you choose “Oregon” for the state, (no matter if the company is in Japan or the system won’t work).
    • For zip code, put in L&C’s zip code of 97219 (otherwise it won’t work).
    • For “primary care provider,” write “None.”
    • For “relationship to policyholder,” choose “Self.”
  11. At the bottom of the form, click on the box in front of “My submission of this form”¦.”
  12. The select “Continue”
  13. When the waiver is confirmed, print out a copy for the student.
